Below, please find links to application forms and requirements. Potential Circuit Bridge Wholesalers and Educators will be required to submit account application and all required documentation. To ensure expedited process of approval, please provide all the necessary information and signatures. Please write legibly and make sure that any faxed/mailed photocopies are clear and legible. Incomplete or illegible applications will not be processed.
Please allow 10 business days for your application to be processed. Upon account approval/activation, you will be emailed the discounted price sheet, along with any necessary account information and instructions. The discounted rates will be automatically applied when using your online wholesaler account.

Our Customer Service Hours
Circuit Bridge Customer Service hours of operation are 9:00 a.m. to 5:00 p.m. PST, Monday through Friday. Circuit Bridge is closed Saturdays, Sundays, and all major holidays. The Circuit Bridge Customer Service department may be reached by calling 408.428.9414, faxing to at 408.428.9411, or by filling out the Contact Us form.
